The idiom "it's an oldie but a goodie" is aptly applied to the use of clomiphene citrate in anovulatory subfertility. This is particularly so in women with polycystic ovary syndrome (PCOS) whereby its anti-oestrogen action is exploited to stimulate production of FSH from the anterior pituitary. Clomiphene citrate has been used as a first-line medical ovulation induction agent since 19671 and it's use remains as relevant today as ever. When "evolving towards excellence" one must be mindful of the various contributors towards the presenting problem in the patient and apply a multi-faceted treatment plan whilst taking care not to over-service the patient. This "fast and furious" presentation of clomiphene will focus on the essentials of its use in clinical practice, tips and tricks, pitfalls and when to move on to other treatments.
